My son wanted to dress as Harley Quinn from DC Superhero Girls for Halloween. Only problem is the costume didn't come with a mask. So I made one for him.
The print is actually printed flat. Then I hit it with a heat gun until it reached the glass transition temperature and became flexible. Then I molded it to my face (because I'm not going to press hot plastic to a four-year-old's face), and added a foam sheet backing for a little comfort.
I also recognize it's a million times easier to Cricut and heatform something like this, but I don't have a Cricut, and if you're here, neither do you! So get over it!
Sample was printed in Hatchbox Black PLA on a Prusa i3 MK3s with the textured bed. .2mm layer height, and I think the filament's a little moist, so some of the layers show in the photo, but it's pretty good on the actual print. I thought about doing some post-processing, but he liked that it was "sparkly," so there you go.
